Full Job Title: Temporary Supported Pathways Co-ordinator – Housing Support

Basic DBS disclosure required
To assess single customers with support needs and create supported housing pathways in accordance with the Homelessness Act 2017.
To work in partnership with the supported housing providers to create move on plans for customers that are ready for independent living.
To ensure that officers use the Council's supported housing database are supported to deliver correct and appropriate referrals.
To liaise with key partners and stakeholders and ensure that additional support such as mental health, drugs and alcohol recovery are provided for customers that are in need.
To work with the Private Rented Sector and Financial Inclusion Teams to ensure single customers with support needs understand their rights and responsibilities along with possible benefits that are available to this cohort.

- Knowledge:
- Thorough understanding of inner city housing issues and in particular Rough sleeping and Supported housing provision
- Awareness of issues and challenges facing rough sleeping and solutions to eliminate Rough Sleeping
- Knowledge of housing benefit rules and regulations, welfare benefits and issues relating to living in supported housing - desirable
- Experience:
- Experience of providing an effective engaged face to face service to vulnerable members of the public and the voluntarily sector - desirable
- Experience of leading or working with others to deliver results
- Experience of monitoring and /or delivering record keeping to a high standard
- ICT Skills: IT skills including Microsoft packages or equivalent (e.g. G Suite)
