A Strategic Change Manager is required to assist the Integrated Director of Commissioning in the implementation of the integrated commissioning approach for adults/children across the Council and the NHS, which enables strong partnerships and high-quality services that support Greenwich residents to lead healthy, fulfilling and independent lives.

To support senior leaders and staff through partnership and co-production; continued design and implementation of integrated commissioning framework; incorporating appropriate governance structures which support integrated, outcome focused approaches to health and care; working collaboratively with the market and residents; leading staff across the system to implement transformational change including further integration of health, care and other forms of support; overseeing the delivery of approaches to commissioning that address personalisation and whole person pathways.

Key responsibilities:

  • Be aligned to one or more strategic change programmes which aim to trailblaze the new integrated commissioning approach. These will be transformational in nature and provide the opportunity for the post holder to bring their passion and enthusiasm for improving both outcomes for residents, improve and modernise working environments for practitioners and ensure best value and impact from health and care community-based services.
  • One of the change programmes for the post holder to play a key role in is the development and delivery of a new integrated Assistive Technology Enabled Care (ATEC) service. This is in partnership with the sector leading Digital and Customer Service team. This is the first integrated ATEC model of its kind to be implemented which enables many opportunities for the post holder to bring existing skills and knowledge as well as to learn new and ground breaking ways of modernising health and care.
  • Be the Interim Strategic Change Manager with responsibility for leading the implementation of the new organisational structure and new ways of working across portfolios for example: Ageing Well, Home First, Early Help, Working Age Adults with Disabilities and Complex Care / MH / Autism Adults, SEND, Mental Health and Emotional Wellbeing Children.
  • Horizon scan and keep abreast of the latest legislation and leading best and next practice in commissioning and share with team members.
  • Accountable to senior leadership for: facilitation of the co-production and implementation of integrated commissioning framework; service improvement initiatives across adult/children’s care and NHS services; effectively managing change as a strategic system leader; implementing the programme plan for the new organisational structure and improved ways of working. This will include facilitating the next stages of the recruitment process alongside other colleagues.
  • Oversee the development of commissioning strategies that are person-centred and strengths-based. Enable staff to work collaboratively with providers to co-produce pathways and design/re-design services.
  • Working with a diverse care market / care providers to enable them to understand the benefits and impacts of the new service, how they will support it to thrive when embedding ATEC within care and support offers and highlighting areas they will need support with to do so: awareness raising, implications for them, opportunities to get more involved.
  • Through managing budgets ensure the delivery and effective implementation of Section 75 and Section 256 arrangements, working with colleagues to co-produce pricing models where needed. Oversee cross-organisation budgets and lead on the implementation of work to improve better use of public sector resources.
  • Collaboratively work with stakeholders and operational teams to review the current service and demand, flow and pathways and undertaking gap analysis. Oversee improvements to service quality and people’s outcomes, ensuring a return on investment.

Qualifications:

  • Educated to post-graduate diploma level in relevant subject or equivalent level qualification or significant experience of working at a similar level.
  • Evidence of continued professional development.

 

Experience:

  • Substantial experience of facilitating and leading co-production and innovation across all commissioning functions.
  • Experience working with digital and technology solutions.
  • Experience of the delivery of highly complex transformational change programmes and associated projects including pilot projects, which includes business case development.
  • Experience of facilitating the co-production of an implementation/procurement route and plan with stakeholders.
  • Substantial experience of quality management and the development of quality characteristics which guide how services will be provided.
  • Substantial experience of working within the financial framework of health and care statutory organisations, senior level experience of budget setting and budget/resource management including of joint commissioning or shared budgets.
  • Minimum of 12 months recent experience in a similar role or context and ideally within a public sector organisation.

 

Knowledge:      

  • Extensive knowledge of health/care policy and practice along with experience of its implementation gained in a health, care or local authority setting.
  • Sound awareness of the needs of vulnerable residents e.g. MH/older adult MH/Autism/Complex Care needs/ PD/ Sensory/LD / older persons/those who are ageing /Children and young people/SEND.

 

Skills & Abilities:

  • Empathetic leadership style, showing passion and commitment, self-motivation, persistence and resilience.
  • Ability to understand the use of data within health and care to support care and support planning, build independence, reduce reliance on services and empower residents and their families.
  • Excellent interpersonal, negotiation and influencing skills and ability to develop and maintain credibility and confidence of colleagues, both internally and externally.
  • Strategic thinking and planning with the ability to explain complex commissioning or policy matters in a clear, compelling and collaborative way to different audiences and stakeholders and across organisational boundaries.
  • Problem solving and prioritisation skills.
  • Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
