The purpose of the role is to provide an effective and efficient high standard of administrative, practical and financial support to the Transitions Team Manager and Transitions Team, within the Learning Disability Service, Adult Social Care.
Key Responsibilities:
- Undertake administration work to facilitate and support the Transitions Team.
- Work and liaise with staff in Adult Social Care as well as staff in various teams in Children, Schools and Families Department including Special Educational Needs Service, Children with Disabilities Team, Short Breaks Team and Looked after Children Team.
- Lead on the coordination and organisation of team meetings and conferences, including providing agendas, taking minutes and be responsible for the preparation of documents for internal and external meetings.
- Prepare and distribute agendas, presentations and papers and take minutes where appropriate for larger meetings involving a number of staff and/or external customers.
- Keep accurate records and process all documentation in relation to Transition case records in a timely manner in Excel.
- Maintain service databases e. g. Mosaic for the Transitions Team.
- Provide detailed minutes and action plan for the weekly Transitions Outcome Forum.
- Provide efficient and effective assistance with projects, complying with all corporate policies and procedures.
- Maintain accurate service monitoring databases to ensure robust statistical information, including inputting sensitive information onto Mosaic.
- Maintain an accurate record on Excel of Transition cases dealt with by the Transitions Team and with other sections within Adult Social Care and teams in Children, Schools and Families including Special Educational Needs, Children with Disabilities etc.
- Organise, coordinate, attend and minute the weekly Transitions Outcome Forum where detailed notes of Transition cases will be required. Provide accurate documentation in an accurate and timely way following forum decisions.
- Organising the Transition Team’s duty rota and office attendance rota.
- Taking calls into the team and directing to the appropriate members of staff as required. Ensuring a clear and professional response to calls and enquiries and taking precise and clear telephone messages.

Experience:
- Minimum of 12 months recent experience in a similar role, or context and ideally within a public sector organisation.
- A high level of experience of complex SEND administrative processes and their management, including the ability to use reports and databases accordingly.
- Experience and knowledge of working with Children and Young People with SEND and their families, and an understanding of the requirements of working with young people with SEND, and schools, settings, agencies, families, carers in a professional capacity.
- Experience working in a multi-professional environment, and coordinating responses accordingly, with the ability to work as a member of a team, sharing workloads with others, taking responsibility to maintain whole service priorities. Alongside the ability to work flexibly and manage a caseload, prioritise and meet statutory deadlines.
- Knowledge and experience of the SEN Code of Practice, current SEND processes and legislation, with understanding of the anticipated changes in legislation.
Skills & Abilities:
- Ability to work under stress and when meetings are arranged at a very short notice.
- Ability to deal with sensitive, confidential and sometimes distressing information
- Communication skills – ability to engage with departmental colleagues and outside agencies and making sure that the meetings will be quorate and all the paperwork available for discussion.
- Ability to take notes “live” straight into a laptop or work effectively from paper notes and making sure the notes are eligible and accessible for colleagues in case of sickness or other unexpected absence.
- Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
