To broker all care provision as specified by Health & Social Care professionals.
Undertake brokerage duties and support the service areas as requested by the Lead Broker Co-Ordinators.
Key Responsibilities:
- Be responsible for the assimilation and distribution of information relevant to the brokerage function, to internal and external providers. This will include contacting providers in respect to commissioning a determined care provision, brokering a difficult care provision, undertaking all necessary liaison with stakeholders, and making required arrangements for the commencement of the care provision as detailed within brokerage procedures.
- Support in the delivery of reablement services, liaising with providers and professionals, ensuring that care provision is transferred effectively between all types of care providers and reablement services.
- Liaise on a day -to- day basis with all operational sections including Social workers/Social Care Workers, Health
- Professionals and Providers and establish effective working relationships, both with internal and external partners.
- Record provision of care as per brokerage procedures.
- Assist in monitoring, reviewing and evaluating the quality of the services provided. Provision of regular information regarding quality of service. Dealing with queries and problems that arise in a timely manner to adhere to required deadlines. Identifying any service improvements, which would enhance the quality of the services provided.
- Ensure compliance with the Council’s recording standards.
- Provide responsive and flexible assistance to Lead Broker Co-ordinator and undertake any tasks to support and facilitate their work as may be required.
- Act as the first point of contact for Providers in respect of delivery of services.
- Co-ordinate any issues relating to the delivery of care provision and escalate appropriately.
- Liaise with finance and the Contracts and Commissioning Unit in respect to services commissioned
