An experienced Adults Social Worker is required to support clients in the Transitions team. To manage a case load and complete assessments, support plans and reviews with adults. Experience of working with challenging client groups is essential for this role.

Key responsibilities:

  • Manage a case load and complete assessments, support plans and reviews with adults.
  • Complete Court of Protection documents, best interests and safeguarding investigations.
  • Assess risk, explore suitable placements (Supported Living, residential and housing) alongside health professionals.
  • Support the team and to ‘hit the ground running’ with experience and knowledge of working with challenging client groups.

Qualifications:

  • A qualified and registered experienced social worker.
  • Social Work qualification (MA/BA Honours, DipSW, or equivalent) and ASYE fitness to practice certificate.
  • Current registration with the Health Care Professional Council (HCPC) maintaining registration and adhering to HCPC standards and code of ethics.

 

Experience:

  • Experience of working with challenging client groups is essential.
  • Significant experience in statutory services with strong communication, engagement and analytical skills to allow you to safeguard and engage effectively with a wide range of agencies.
  • Liquid Logic experience desirable.
  • Experience of writing high standard reports and written records for a variety of purposes including those in a legal context.
  • Experience of managing a high level, complex and diverse caseload within agreed parameters, policy and procedures.
  • Experience and skills in multi-agency partnership working.

 

Knowledge:

  • Sound knowledge of relevant legislation.

 

Skills & Abilities:

  • Able to apply consistently high standards to ensure better outcomes.
  • Self-motivated, innovative and creative.
  • Ability to prioritise work with competing demands, frequently changing demands.
  • Ability to communicate, negotiate, challenge and manage conflict, acting respectfully when people are resistant to change, managing tensions in ways that show persistence, determination and professional confidence.
  • Ability to work as an autonomous professional, exercising own professional judgement using written guidance and established procedure, recognising one’s own professional limitations and how and when to seek advice from a range of sources, including practice supervisors, senior practice leaders and other multi-agency providers.
  • A high level of effective written and verbal communication and the ability to pass information accurately and concisely with a range of partners.
  • Strong IT skills including Microsoft packages or equivalent (e.g. G Suite).
